Not only your action is off balance but off direction too.

For starters, your upper body weight is not transferring on your left leg on time. And hence you are trying to generate some speed, mostly from your shoulder.

You can take two routes from here.
Remove the jump from your action and adapt the bowling action as in the likes of Harris Rauf, OR, you can adjust your action by trying different ways to transfer your weight on your left leg when you land a little early and make your right shoulder muscle work insync with your left leg.

Another way to practice is, just practice throwing. Aiming at the stumps as if you are sending it down to make a quick runout and throw the ball by transferring all your weight on your left leg.

Second, with this action, the line of your delivery is way too predictable. You will almost always bowl in-coming deliveries. Outswing is almost impossible with this action.

Use the crease and get as close as you can with the stumps and send the ball towards off stump and slips.

And finally, it won't hurt to shed off a few extra pounds.
