Need urgent feedback from a Spinal Doctor/Specialist on Pakpassion

I am attaching the findings of an MRI i had done a few days ago and the Doctor's report i received.

I have had a sharp shooting pain in my right leg in the hamstring area for the last 2 and a half months. I don't recall anything which immediately caused the problem, i suspect it may have been caused by a rock climbing session i did in May though i never felt any pain or discomfort during or after the session. All i remember is waking up one day in May and feeling a sharp shooting pain in my right leg.

I at the time suspected it just might be a niggle, hamstring strain which would go away with the passage of time after resting it naturally and using pain killers. Unfortunately the problem persisting and kept getting worse. I went to my family doctor a few times during this period who kept on treating it as if it was just a minor hamstring strain and he just prescribed me pain killers and later just suggested i go to a sports rehabilitation centre.

Finally a few weeks ago, two doctors, specialists that i went to suspected that i was suffering from Sciatica and they recommended urgent physiotherapy and recommended that i have done an MRI asap. Unfortunately in Canada it is a huge pain to get an MRI done, to see specialists because the waiting line is huge which means you have to wait for months.

So i decided to come to Pakistan a few days ago and immediately had an MRI done at Jinnah Hospital and i am attaching the results.

Looking at the results, i obviously cannot understand what the results mean but my mom who is a doctor says i cannot weight train, excercise ever again, lift heavy objects ever again in my life which is scary and depressing to hear.

The doctor so far is recommending that i go on medication, bed rest for the next 10 days, minimizing the use of stair cases and the only physical activity that he is permitting me to indulge in is swimming.

Anyways i am attaching a copy of my MRI report and i am hoping for some feedback from doctors, spinal experts on Pakpassion for their layman explanations, feedback for which i will be most grateful

^^^^^ Your mothers assessment is a bit over the top. You have mild/moderate radiculopathy (compression of the nerve root as it exits the vertebrae laterally) between your L5 and S1. You probably suffered a muscle spasm on top of this which resulted in the sharp pain. Continue with the anti-inflammatory drugs and start physiotherapy. FYI my back is even more messed up (I have severe spinal stenosis). I used to have the same pains as you and started working out lightly and built up. I squat 300 and deadlift 400 without any pain at all now.

Thats my POV as well. People with worse injuries, amputees, arthiritis still weight train with adjustments.

In fact i feel if i train correctly, with the correct form and strengthen the muscles in my back, it will improve my back. What i will do is make my personal trainer meet my doctor so that they both are on the same page and that my trainer knows how to train me without putting my back at any further risk.
 
I had a terrible mouth ulcer some weeks back that has mostly healed since then. Problem is that now I find their is to much chilli in almost all main meals especially curries and even plain roti. Can someone tell me why this is and recommend a remedy? My mouth is often on fire when I am eating.
 
I am a patient of polan/dust allergy from at least 2 years. I've checked it from 2, 3 Doctors but the problem still continues. any advice ?
 
Burning mouth

My mouth, lips and tongue burn whenever I eat anything. It started a while back when I ate some salty snacks leading to very bad mouth ulcer's. Where as the mouth ulcer's have healed the burning has not. Any advice on how to heal it?

For symptomatic relief.
Gargles and Rinsing, twice a day, with mixing 10ml each of ;
1= Glycerine
2= Mouth Wash (Enziclor)
3= Lignocaine 4% Solution

Meanwhile You can test for your
1=Haemoglobin
2=SGPT
3= Uric Acid
in that order.

Muscle twitching all over body. Often having poor sleep because of that. Resulting in lack of awareness in day time. Now eyes don't feel good. Went to doc, he did all sorts of blood tests, everything came normal. Any ideas?
 
I have been born with a condition which is quite rare in this world but my great grandfather was also effected by it and it effects my physical appearance but thanks to my friends,family and acquaintances who have never made me feel different from them even though I know that that there are things which I cant do properly.

The credit goes to my family and my childhood friends who never bragged about it.There were some idiots in my middle school who used to make fun of it but my teachers and my friends used to scold them and they realized with time that it is not something to be made fun of

Cure would be in case if you have an illness. MOST cases of Oral Ulceration have association with inadequate oral hygiene in preceding days rather than an illness causing it.

There is a list of diseases that can cause oral ulcers BUT all these conditions cause OTHER TROUBLESOME SYMPTOMS much before giving you oral ulcers. When one doesn't have any other major symptoms but just oral ulcers, it is extremely rare that ulcers are because of an illness.

Assuming that you don't take excessive amounts of Coffee, Tea or Chocolate; have you checked for Calcium levels in Blood ?

What symptoms are those? The ulcers have mostly healed but the burning continues. It occurred after I ate a lot of salty crisps. Was fine before then. Now I find almost everything to contain to much Chille.

I do take loads of chocs and tea now, but it was happening before that.
Yes, calcium and other things came normal. Even the thyroid ones.

If you need surgery I don't see how anything non-invasive is going to fix it.
 
It is difficult to comment without a thorough diagnosis, but generally speaking, the most widely used non-invasive treatment for ligament issues are Platelet Rich Plasma ( PRP) injections.

However, it is an expensive option and may not be effective if your orthopedic feels that surgery is the only optimal option at this stage.

If available, Prolotherapy.
Otherwise Physiotherapy.

I think Low Level Laser Therapy ( LLLT) may have a role but I am not sure.
 
My diabetes has been diet controlled with average test monitoring reading of 6.3 mmol/L (not fasting)
However, as I now have had heart bypass (mainly due to my bad genes from parents) , my GP has advised me to start taking Metformin - but I am not sure whether I should or not as I heard taking this tab has many long term side effects as well. My question to any medical experts on this forum is that would I be doing more harm by not taking Metformin and trying to control it with only diet& exercise ?

Every drug has side effects.

You have to weigh benefit vs side effects.

In your case, genetic predisposition of CAD and cardiac bypass, which is also exacerbated with diabetes, Metformin's benefit out weight the side effects.

Thanks for advise I was thinking alone the same lines when I agreed with my GP to go on the drugs. Hopefully if my sugar level is on lower end of 6 for few months the GP may decide that my diabetes is under control and try taking me off the drug.

Metformin is the most beneficial diabetic medication ever invented for people with heart disease or even to prevent heart disease. Its also one of the safest for people without advanced kidney disease. Your GP is right, you should take Metformin.
 
Help - need your medical advice

my age is 35, my one ear is having pain from last 2 days, its not bleeding or anything, just got milf infection. i have visited the doctor, he prescribed me Tablet Augmentin 375mg Twice a day and Tablet Voltral (Diclofenac sodium) 100 mg Twice a day, he told me to take Augmentin for 5 days, he says i have some kind of mild infection in the ear. i am fine with that.

but my ear pain is killing me, whenever i take one dosage of Diclofenac sodium, my pain goes away in an hour and it wont return atleast in 5 to 6 hours, but after that it appears again and wont stop until i wont take second dose of Diclofenac sodium again.

now my simple question is how many Diclofenac sodium dosage (Tablets of 100 mg) i can take in 24 hours,
doctor prescribe me 2 times in a day, but its not reducing my pain, i have to keep taking it after every 6 hours.
i have to take appointment from doctor again in next 2 days, until thn i dont understand what to do with my situation here.

if someone plz help me. i will be really thankful to them. i have very severe pain in ear and i couldnt get a proper sleep and it wont stop until i take Diclofenac sodium dose after every 5 to 6 hours.

Diclofenac two times is fine, every six hours for long time will make problem for you so better to take Paracetomol for pain and consullt your doctor regarding infection. By the way 375 mg
augmentin is not proper dose. Should have been 625 two times a day at least.
 
You may have infection called otitis media for which sometimes medicines which improves the patency of the tubes connecting the ears with the nose are important and you may need se medications for that.

You can take 300 mg per day for 2 days but you have to take more water to avoid kidney or stomach damage.

Question to doctors

i did not get tested, but i think i had the corona virus. Been almost two weeks. All symptoms are gone other than extreme fatigue , loss of smell and taste. Since yesterday morning i think i have broken out in stress hives. the spread is very severe. i took claritin twice now. the hives are worse today than yesterday. any advice?

at this point, it's better to consult your gf who probably could make the admission and testing in a hospital easier for you. Don't wait at this moment.
 
Everybody has a thermometer at home to check for fever. I think in the current situation it is also a good idea to have a Pulse oximetry at home. It's a simple device to check your blood oxygen level. It can be bought in a typical medical store.

A typical blood oxygen level should be in the range 95-100. Though 100 is not typically achieved. While anything under 90 could be considered as low. However this can vary for person to person.

What I would to do is, establish a personal baseline oxygen level. While you are still healthy and free of any symptoms take multiple measurements through out the day and note them down. This will give you an idea of what you typical level is. You only have to do it once to get a basic idea.

Why does it matter?

Covid-19 gets tricky when the virus start to attack the lungs and damages them. This can show itself in difficulty breathing. However, it's possible that many people don't really notice it in early stage or don't pay much attention to it. Especially the elder folks might have a hard time describing their symptoms or are prone to down play them. This could be potentially harmful, for you may notice it only after significant lung damage has happened and the person is gasping for air desperately.

A damaged lungs has a decreased ability to provide oxygen to blood and this would lead to a lower oxygen level than usual.

If you or someone in the family has the typical Covid-19 symptoms then Pulse oximetry can be used once a day to check whether your level is in your baseline range or if it has gone down significantly. If you are consistently getting low blood level readings then that would be a good indication to call for a medical emergency immediately.
 
Doctors have sounded the alarm on tiny plastic particles entering the human body after finding people with higher levels in blood vessels had a greater risk of stroke, heart attack and earlier death.

Scientists have known for some time microplastics are entering our bodies via the food we eat, water we drink and air we breathe.

Little is understood about the impacts they might have on health, but a new paper published on Thursday could mark a "step forward" for scientists' understanding.

The study found patients with micro- or nanoplastics in the fatty plaque in their blood vessels were much more likely to suffer a stroke, earlier death or heart attack than those whose vessels were not contaminated with plastic.

The researchers identified plastic in the blood vessels almost three in five (58%) of the 304 patients enrolled in the study in Italy.

The results were published in the peer-reviewed New England Journal of Medicine (NEJM).

The findings do not prove the plastic particles caused the heart attacks and strokes, as the patients could have been exposed to other risk factors.

But they chime with other research that found microplastics harm human cells, released by Hull York Medical School and the University of Hull in 2021.

Prof Richard Thompson from Plymouth University's International Marine Litter Research Unit, said: "Our laboratory studies have shown that within a few hours, nano plastics can spread from the digestive tract, via the circulatory system, throughout the body of marine organisms."

He added: "So perhaps not surprisingly there have been recent studies showing the presence of micro or nano plastics in human blood and tissues.

"This paper makes a key advance, helping evidence the potential for associated harm."

Dr Danya Bakhbakhi, clinical lecturer in obstetrics and gynaecology at Bristol University, said the research "could be a step forward in our understanding".

But she said other factors could be to blame for the health problems, such as socioeconomic data, which has been linked to coronary artery disease.

"Further larger studies, in different geographical locations need to be conducted to see if these findings are replicated," she added.

The world is producing more and more plastic, only 9% of which is successfully recycled, with the bulk ending up in landfill, incinerated or leaking into the environment, according to a new OECD report.

Plastic does not break down for at least hundreds of years.

Jo Royle is CEO of Common Seas, a campaigning organisation that in 2022 commissioned peer-reviewed research found plastic in blood.

She called the new research "profoundly worrying, but unfortunately not a shock".

She added: "From our blood to our hearts, plastic has now infiltrated every part of our body.

"Governments globally must now act swiftly to ban harmful plastic products and unite behind a legally binding Global Plastics Treaty that recognises the threat plastic poses to human health."

In 2022, 175 governments agreed to draw up an international, legally binding agreement to end plastic pollution.

It was due to be finalised by 2024, but negotiations are ongoing.

 
We should avoid eating things that are wrapped in plastic. It's high time to reduce plastic usage.
 
Doctor among five held in KP, fake medicines seized

The Federal Investigation Agency has arrested five persons, including a doctor, for their alleged involvement in supplying unregistered medicines to Khyber Pakhtunkhwa and Afghanistan.

A statement issued by FIA said that the agency raided different areas in the provincial capital and Shabqadar tehsil of Charsadda district and seized fake and unregistered medicines worth Rs77 million.

The statement said a raid was carried out in Gulbahar area of the provincial capital, where the agency recovered fake and unregistered medicines worth Rs43.5 million on the lead provided by the alleged suppliers.

The statement said a doctor was arrested following initial investigation from the accused persons. The agency also recovered medicines worth Rs28 million from the doctor’s house, also in Gulbahar area.

“Raids were also conducted in Shabqadar, where fake medicines worth Rs5 million were recovered from different godowns,” the statement said, adding that a case was registered against the arrested persons.

The statement said that the case would be probed further in light of the report of the Drug Regulatory Authority of Pakistan after examination of the seized medicines.
