Are Pakistani physicians treating Covid-19 patients with dexamethasone?

Was wondering what treatments physicians in Pakistan are giving Covid patients. Apparently Dexamethasone has been proven the most effective in treating COVID and reducing risk of death, it's what was used to treat 74 year old Donald Trump. Are Pakistani healthcare workers aware of this?
 
Unfortunately a lot of people in Pakistan are dying and this is mainly cause of the poor healthcare system and ignorant and unprofessional medical personnel.
 
Pretty sure they have been for months. From what I remember the studies showed that Dexamethasone is most useful in people who are quite severely ill from Covid. Some of our Pakistani doctors may be able to confirm [MENTION=131701]Mamoon[/MENTION] [MENTION=140234]DRsohail[/MENTION]

The people might be poor and uneducated, the doctors are not. Part of the reason for Pakistan getting through the first wave with relative success was due to our fantastic medical staff making the most of the limited resources available to them

Yeah we are all ignorant and have no idea what we are doing. We were not aware of dexamethasone before you started this thread.

If you would have done a bit of research before opening this condescending thread, you would have learned that Dexamethasone (Decadron) is locally manufactured in Pakistan and is regularly used to treat COVID patients.

One injection costs around Rs. 530/- which is about $3.

Pakistani doctors and paramedics are highly competent and professional. It is one of the major reasons why Pakistan has been able to deal with the pandemic reasonably well in spite of the idiocy of the masses and the confused decision-making of the government.

I bet you have never been to a hospital in Pakistan and have never been treated by a doctor in the country. Please do your research before making blanket, ignorant statements.

This is a big insult to the medical profession in my opinion. This just shows how much out of touch with non native Pakistani are in the subject of pakistan.

To OP, in every disease, there's always a protocol about how to diagnosis, and then comes drugs with their dosage with added surgical if there is any requirement. Every hospital in all corners of the world regarding covid has got the memo by now. Due to expanse, some patients may not be able to afford it in some areas but that doesn't mean medical professionals don't know about the protocol.

Don;t know what's your motive behind asking whether Pakistanis doctors know about a very basic thing about corona virus treatment. If a non-doctor like you knows it, you should have no doubt about the competency of Pakistani doctors who are as good as doctors anywhere in the world. No wonder Pakistan was among the countries in the world with the least mortality rate during and after the first wave in summer.

And if you are a doctor yourself, you should have known that Trump was not just given dexamethasone, he was also given Remdisivir and most importantly Drugmaker Regeneron's dual-antibody cocktail
which I think made the most difference. That antibody cocktail was not approved for public use at that time, and only got approved yesterday. Similar antibody cocktail Bamlanivimab made by another company was approved by FDA on Nov 9 and we are using it here in USA and is very effective.

Dexamethasone is not the main treatment for corona virus infection.

Doctors in Pakistan know all about it but obviously they can treat the patient with the resources available to them .

Yes that's expensive,in India we will get Dexamethasone at Rs 40 and sometimes even free if you have relatives or close ones working in the hospital and in pharmacy store.

Dexamethasone is a corticosteroid. It won't treat the infection but may help alleviate some of the symptoms. For instance, it can serve as a bronchodialator, preventing the need to put the patient on a ventilator. It may also control the cytokine storm-related viral sepsis that is actually causing most of the COVID-19 deaths, although the evidence for this is quite thin.

OUR consultants were using it even before it was started by other countries. We are ahead in introducing this treatment.

Also it's no use to start a treatment without also doing RCTs. How would one even determine that it's useful? So the question is if we introduced it so early, why weren't we doing those trials?

If it weren't for the NHS, we and the world would never have known that the drug is actually saving lives.

Thanks for addressing the ignorance of the OP because even I know Dexamethasone is used in Pakistan to treat those suffering from COVID.

In the end, all RCTs mention to individualize treatment according to clinical assessment. And there is no harm in listening to clinical approach of a Consultant Pulmonologist even if their are no prior RCTs.

Thanks to one of Pulmonologists, Dexamethasone was being used in our hospital way before the NHS recommendation.

Trials part I agree with you. We don't have enough funds/culture of conducting clinical trials. But clinical reasearch has started to get more and more emphasis in last 5 years.

Dexamethasone has been used against asthma and other respiratory conditions for decades now.

It is more likely that they are using it to relieve breathing difficulties in COVID-19 patients, than against the virus itself.

As I mentioned earlier, dexamethasone is NOT a treatment for COVID. It's merely a symptomatic treatment for the associated breathing problems.
 
RECOVERY trail confirming the usefulness of dexamethasone came out in July, physicians all over the world were using steroids months prior to that. RECOVERY trial just confirmed what every one was expecting, but it was a very important trial.

Dexamethasone is not the only steroid being used , this was used due to its convenience, its long acting and needs to be given once daily. Other steroids like Methyprednisone are equally effective, juts needs twice daily dose.

It was mostly bon individual trier basis . The most important thing we are missing here that we did not do a collective study although most of the consultants were aware of the good results in RDS patients.
