There is no discipline in his lifestyle and no focus. He has had 2+ years to get into shape and he is refusing to change his diet.
He is also not interested in FC cricket.
Cricket appears to be a hobby for him. A leisure activity. The only reason he is playing at the international level and PSL is because his father is Moin Khan.
If he doesn’t mend his ways, he will play T20 cricket for a 2-3 years and then vanish into oblivion because there is only so much that his father can do for him.

In Pakistan hype sells, especially if it's a hype about someone's son who is connected like moin khan and there is a whole mechanism of daddy's friends, ex cricketers from the same city, journalist who call themselves cricket journalist, and have TV shows, where are these clips of him scoring few boundaries here and there are going to be shown over and over again, and most importantly the loyalty to the city, is more important than anything else.
Pretty soon this guy is going to be in a T20 squad, they're just waiting for the right opportunity / right opponent(ideally weak team) to field him against, and his expectations are to score good two three hits in an innings and then he will become a permanent fixture of the national T20 squad.

A neurosurgeon has recommended an observation period of 24 hours. He will then be reassessed on Monday.

He does not want to lose weight because he will lose his power hitting, that is baloney, if you lose weight the healthy way, and convert your fat into muscle you will not lose your power hitting ability, if anything it will be better
Regarding his keeping since he is so pick can barely bend down, and has really slow reflexes, why is that.....because he is too fat (i am saying that nicely, not in an insulting way)
He losses weight he will be a better batsman, a better fielder and a better keeper, and cricket aside his general health in his daily life will be better as well.

He should be available for selection ONLY AFTER he is locked in prison for a year.
And during this year, they should give him controlled amount of low calorie and high protein diet with lots of hydration. And make him do nothing else but daily gym and running for muscle fexibility, weight training for muscle building and muscle strength, and sufficient enough amount of sleep for his body to recover. No other activity. No internet, no TV, no phone but yeah, he should be allowed to use restroom and showers.
His day should start at 5:30 AM and end at 7:30 pm - No exceptions for one entire year!
After a year, he will have an opportunity to solidify this discipline in his day to day life - that he SEVERLY lacks at this moment.
Currently, at a single time he eats a portion size of four people's worth of uncontrolled calorie diet - no kidding!
Do not know if your being sarcastic, but he has to find a way to lose weight, and build fat into muscle, a lot of people have and can achieve this, and I believe he can as well once he is serious enough.

Nobody is stopping him from playing club cricket and t20 leagues but a young kid with that size has no place in international cricket.

Lessons should have been learnt from the Umar Akmal experiment.

Not having a good keeper costs you immensely when your only consistent strength is your bowling attack. In tight games, things like catches, stumpings, byes and runouts can make all the difference. And if your keeper ain’t up to speed, you lose those marginal gains.
But people get dazzled by some runs in the PSL Vs QG.
This is why his weight is an issue. I could care less about aesthetics, as long as a player is effective. Azam is not an effective wicket keeper behind the stumps at this level. His lack of fitness gets in the way of his reaction times.
I think most nations would give him a fitness plan, get him working with the trainers and give him a period of time to reduce his weight by.
If he hadn't met the weight goals and fitness targets by that allotted timeframe, then he wouldn't be selected for the national side.

Look at Usman Khawaja, he already looked in not so bad of a shape but in his youtube video, he clearly says that the coach Justin Langer told him straight up, to lose 10 pounds and be fitter, or you are out.
